What is the Adult Placement Scheme for?
The Adult Placement Scheme helps people:

who prefer to live as part of a family, rather than in the community

who want to develop their independent living skills before moving into Supported Living. This includes young people moving from children's to adult services

whose own family or carers want a break.
Who can use the Scheme?
You can use the scheme if you:
- live in Nottinghamshire
- are aged 18 or over
- have been assessed as eligible to receive learning disability services
- have a moderate to severe learning disability.
What are the carers like?
They all have different backgrounds and lifestyles which means you can find someone you are happy to live with.
All the carers are trained to a high standard and have the knowledge and skills to help you live as independently as possible.
How much does it cost?
The Adult Placement Scheme offers a number of support services. Some are free and some you will need to pay for. You will know beforehand how much everything will cost.
How do I decide if I do want to live with a carer?
We will talk to you and the people important to you to find out what you like and what support you need. This helps us find a carer who you will be happy living with.
You can visit the carer several times before you make a final decision that you want to live with them.
How do I know I'll be happy and safe in my new home?
You will have a co-ordinator who you can talk to at any time about anything you are worried about.
All carers are assessed and monitored to make sure you get the best place to live as possible.
The Adult Placement Scheme is inspected by a national body called the Care Quality Commission (CQC) who make sure everything is ok.

How can I find out more?
Get in touch with your nearest Community Learning Disability Team(CLDT) and ask about the Adult Placement Scheme or ask to speak to the adult placement co-ordinator.
Someone from your CLDT will visit you to talk about your needs. If you still want to find out more after this an adult placement co-ordinator will get in touch and discuss it further.
