Short-Term Personal Support

Short-term support is for when you need that little bit of extra support to get on with life. It is especially suitable for:

  •  older or disabled people who feel vulnerable or have lost confidence living at home
  •  people who have had an accident or illness that means they struggle to do the things they used to do
  • when you have been discharged from hospital and need a bit of extra support to help you get independent again.

 

 

Community support

There are many organisations in the community that can offer short-term support. This could include personal care and help with practical jobs around the home. See our community support page for more information.

 

Support from Nottinghamshire County Council

The Short-Term Assessment and Re-ablement Team (START) can work with you to regain the skills and confidence to live as independently as possible. It can help with tasks such as getting dressed, using the bathroom, getting around your home and cooking.

If you think you need short-term personal support from the council contact us and tell us about your needs. You can do this yourself or get someone else (such as family, friend or GP) to do this for you.

If you are in hospital ask to speak to the hospital social work team.

If you need any help to contact us our advocacy services might be able to help.

Staff will visit you to:

  •  discuss the kind of support you need and how we can help you 
  •  agree the support we can offer you in a support plan 
  •  work with you to help you achieve what’s been agreed in your plan.

 

Some of the things START staff may do: 

  •  suggest doing things in different ways to how you have done them in the past
  •  offer small items of equipment to make tasks easier
  •  let you know about other kinds of help you could get and different ways you can pay for it.

 

This support can last for up to six weeks and is free of charge. During this time your support will be constantly reviewed to see if you need any long-term personal support.

 

Hospital Discharge Schemes

There are several schemes in Nottinghamshire for people who need short-term support when they leave hospital. All schemes are different but they will all help with immediate practical support with tasks such as:

  • collecting prescriptions
  • shopping
  • light domestic chores
  • preparing your home before you are discharged.
